Job Description/Job Purpose: The person in this position will facilitate child and family teams to plan for the provision of services to families and children. They will also perform duties of the evidence based clinical practice of high-fidelity wraparound facilitation in accordance with state expectations and provide coordination of services and supports for families that qualify for Children’s Mental Health Initiative/Children’s Mental Health Wraparound.
Responsibilities/Duties:
- Complete a comprehensive assessment while working in full partnership with team members to develop a full plan of care
- Demonstrate ability to assess the immediate needs in the different ages and groups including those with disabilities
- Establish appropriate clinical relationships with clients to obtain data needed to complete assessments
- Develop plan of care for services, discuss/plan/negotiate with team members and other providers of services and to effectively community plan to family or to make appropriate referrals where needed
- Develop family timeline, family story, family vision, functional strengths, identifying and selecting natural supports, personal need statements for all family members, family mission, action steps to meet needs
- Demonstrate the ability to seek out, evaluate and use other sources of assistance for the individual and their family
- Follow up with referrals to ensure the individual and family have obtained the necessary services and advocate for them as necessary
- Facilitate in-person child and family team meetings (CFTM) based on individual needs and strengths face to face, no less than once a month at times convenient to the family and team members involved
- Knowledge and demonstrate elements of Crisis Intervention
- Understand social services delivery system to select appropriate resources and effectively communicate with the appropriate resources
- Provide in-home visits to individuals and families for supportive and educational purposes as determined by plan of care and/or care team
- Coordinate services, facilitate multiple service providers to work as a team on behalf of the individual and their family
- Completion of a plan of care with the Wraparound Team outlining the participant’s needs and entities responsible for addressing
- Oversee implementation of plan of care
- Assure care is delivered in a manner consistent with strength-based; family driven and competent values
- Frequently interact and communicate with therapists, physicians, community agencies including DCS, court, schools, health care providers and other local agencies
- Offer consultation and education to all providers regarding the values and principles
- Monitor progress toward the treatment goals; monitor all services authorized for a child’s care
- Ensure that necessary data for evaluation is gathered, recorded and reported to DMHA as per requirements
- Participate in team meetings for business development and program planning
- Keep abreast of current knowledge, best practice, industry standards
- Ability to speak and present information in front of groups of people
- Effectively communicate (written and verbal) and work with team members and executive team
- Have outstanding proficiency in computer software including Canvas, State and Agency required software, Microsoft Outlook, Word, Excel, Power Point, TEAMS, Adobe PDF, Dropbox, file downloads and uploads, print to PDF, file conversions and accessing online resources.

- Maintain required equipment (computer, internet, phone, printer, etc.)
- Other Duties as assigned to redevelop presence and purpose of this position with Advocacy Links
Qualifications and Requirements:
- Four-year degree (or higher) in Human Service field (psychology, social work, sociology, etc.), required
- At least 2 years of experience working with seriously emotionally disturbed (SED) youth and their families
- Verifiable identity and eligibility to work in the United States and must be a minimum of 21 years of age
- Valid Driver’s License and active liability insurance
- Maintain clear criminal history
- Negative results from five panel drug screening
- Current 1st Aid/CPR certification, preferred upon hire but not required
- 20 hours/year of ongoing training specifically related to their field, Federal & State Benefits and incentives; Disabilities; Mental Health, Child Development; etc.

Employment Status:
Travel Required: Travel to meet with individuals, families, community members, sponsors, and vendors, etc. to provide services in a localized area, which may include up to 60 miles from home office.

Salary / Caseload: (caseload of 10-12 across 3-4 counties)

Possible salary increase up to $45,000/ per year after all initial Advocacy Links and State required Facilitator training requirements are met; expected timeframe 7-8 months from hire date.
