Job Summary
We are seeking a skilled Nurse to fill the position of Healthcare Liaison in our Home Health department. As a key member of our team, you will be responsible for coordinating patient care and ensuring seamless transitions between healthcare settings.
Key Responsibilities:
- Screening and Referral: You will be responsible for screening patients to determine their eligibility for home health care services and referring them to the appropriate programs.
- Care Coordination: You will work closely with patients, families, and healthcare providers to develop personalized care plans that meet their unique needs.
- Advocacy: You will advocate for patients' rights and interests, ensuring they receive high-quality care and support throughout their treatment journey.
- Documentation: You will maintain accurate and comprehensive documentation of patient interactions, care plans, and outcomes.
Requirements:
- Licensure: Current RN licensure is required.
- Experience: A minimum of 3-4 years of experience in nursing, preferably in home health or a related field.
- Skills: Excellent communication and interpersonal skills, strong critical thinking and problem-solving abilities, and proficiency in electronic medical records.
