Job Title : Registered Nurse

Experience : 1 year of Home Health RN experience

License : Active state RN license (IL)

Certifications : Active BLS (AHA)

Must-Have :

Wound care experience (wound vacs, pleural drains, and ostomy care)
Ability to complete peripheral blood draws, central line care, and IV administration through multiple devices (elastomeric, IV pumps)
Excellent assessment skills
Experience with computer documentation
Reliable transportation, active driver's license, and insurance

Job Description :

The position is 100% field-based, requiring the nurse to drive from one patient to the next, with no office to report to, and meetings held virtually.

Initial training is required on-site for 2-3 days.

The Case Manager can expect to see 20-30 patients per week, with an expectation of 6 routine visits per day.

Patients typically have multiple comorbidities, including (but not limited to) CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, fall, fracture, infections, cellulitis, post-surgical needs, wound care, ostomy care, labs, and IV infusion.

Care includes education on medications, disease management, self-care, and performing skills such as wound care, infusion, line maintenance, and ostomy care, along with teaching patients how to perform these types of care.

Utilizes EPIC / computer charting and Sapphire IV pumps for care documentation.

The traveler will be provided a laptop for charting.

Any solid color scrubs are acceptable.

The coverage area includes the following zip codes and surrounding areas : 60630, 60618, 60625, 60646, 60659, 60660, 60657, 60640, 60613, and 60614.

Candidates must provide their own transportation to and from all patient visits.
