Job Description

Job Title : Registered Nurse

Job Specialty : Home-Hospice Care Operations

Job Duration : 13 weeks

Shift : 8-hour Day Shift, 40 hours per week

Guaranteed Hours : Not specified

Experience : Minimum of 1 year Home Health RN experience

License : Active state RN license

Certifications : Active BLS (AHA)

Must-Have :

Wound care experience, including wound vacs, pleural drains, and ostomy care
Ability to complete peripheral blood draws, central line care, and IV administration through multiple devices (elastomeric, IV pumps)
Excellent assessment skills
Experience with computer documentation
Reliable transportation, active driver's license, and insurance

Job Description :

This position involves 100% fieldwork, with nurses traveling from one patient to the next

No physical office to report to; all meetings are conducted virtually, and nursing supplies are delivered to the nurse's location

Initial training required on-site for 2-3 days

Case Manager is expected to see 20-30 patients per week, averaging 6 routine visits per day

Patients typically have multiple comorbidities, including but not limited to CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, falls, fractures, infections, cellulitis, post-surgical needs, wound care, and ostomy care

Responsibilities include education on medications, disease management, self-care, wound care, infusion, line maintenance, and ostomy care; educating patients on performing self-care is also required

EPIC / Computer charting system in use; Sapphire IV pumps

Travelers will be provided with a laptop for charting

Any solid color scrubs are acceptable

Coverage area includes various specified regions

Candidates must have their own transportation for all patient visits.
