Job Title : Registered Nurse

Job Specialty : Home-Hospice Care Operations

Shift : 8-hour Day Shift (40 hours per week)

Experience : Minimum 1 year of Hospice RN experience

License : Active RN License

Certifications : Active BLS (AHA)

Must-Have :

Strong communication skills with providers and the ability to educate patients and their families
Knowledge of pleurx drains, nephrostomy tubes, trachs, PCA's, and gastric suctioning (troubleshooting knowledge required; training provided if needed)
Active driver's license, auto insurance, and reliable transportation to / from all patient visits
Highly preferred experience with EPIC, though not required

Job Description :

This is a remote fieldwork position in Hospice Home Care.

Staff will primarily see patients in community settings, including their homes, family members' homes, Assisted Living Facilities, Group Homes, and Nursing Homes.

The RN Case Manager's caseload is 22-25 patients per week, with 5-6 visits per day.

Expect to receive up to 2 weeks of orientation, depending on comfort level.

Wear any solid color scrubs without logos or patterns; closed-toe shoes are required.

Advocate Laptop, cell phone, and PPE will be provided.

Coverage area includes the Southside of the city, from the lake to roughly Pulaski Ave.
