Overview: The objective of the Community Relations Director is to lead the admissions process to ensure that all active referrals are effectively communicated, coordinated and completed in a prompt and professional manner. Generate referrals and promote growth in both shared housing and traditional home care. When necessary, intervene in the admissions process to expedite referrals and admissions.
Professional Responsibilities:
1. Effectively lead the admissions process while being a forward-thinking leader that invests in the development of your admissions team (i.e. admissions coordinators.)
2. Establish, nurture and maintain constructive professional relationships with all referral sources.
3. Continuously search for opportunities to expand home care and shared housing through targeted marketing calls and visits to referral sources; formal and informal marketing efforts in key areas and forums within the community; and consistent communications with service coordination entities.
4. Maintain a positive, stable approach to all work and communications. Ensure all communications both written and oral are respectful and professional regardless of who it is with.
5. Quickly become involved, both directly & indirectly, in the admissions process any time that consumers face obstacles that delay admissions.
6. Maintain a robust funnel to ensure that sufficient referrals are aligned with projected need and/or growth. Also, maintain an up-to-date and accurate marketing/sales spreadsheet in order to facilitate referral and admission discussions.
7. Actively lead routine admissions/reporting meetings to provide updates, set target dates for admissions and any other relevant information that should be communicated.
8. Maintain an accurate, detailed and up-to-date admissions spreadsheet, making updates as circumstances change with referrals on a daily basis. Identify trends and areas for improvements, while productively and collaboratively work with the CEO and the Operations team to implement agreed to changes.
9. Serve as a solution driven advocate for consumers and drive efficiency throughout the admissions process. Partner consistently with the operations team to ensure effective communication within internal departments.
10. Partner closely with the CEO & VP of Community Health Choices to strengthen relationships with Managed Care Organizations, ensuring we are the provider of choice.

Job Type: Full-time
Pay: $80,000.00 - $90,000.00 per year
