What are the responsibilities and job description for the Vice President, Community Health Engagement position at Affinia Healthcare?
POSITION SUMMARY: Perform the duties required to be responsible for the oversight of community health-related outreach and delivery programs and patient engagement initiatives. Provides leadership in the oversight and improvement of clinical operations to ensure quality, patient-focused service delivery. Responsible for developing and achieving targets in annual business plan, fiscal management, creating and monitoring expense and revenue budgets, participating in public grant reporting and audits, and program monitoring.
POSITION QUALIFICATION REQUIREMENTS
Education: Master's Degree preferred in health-related field.
Experience: Eight (8) years of related and/or applicable experience, five (5) or more of which must be in a director level capacity in an ambulatory care outpatient facility. Positions with increasing responsibility including the management of staff and projects are required.

FUNCTIONS OF THE JOB
Essential Functions
- Functions as the strategic leader for community health, outreach, and patient engagement initiatives.
- Shared responsibility for establishing highly effective relationship with Senior Leadership to ensure alignment of clinical operations processes.
- Oversees the coordination of community and maternal child health, PCMH, outreach, school-based health, mobile health, immigrant and refugee health, WIC, unhoused services, practice transformation, and Gateway to Better Health initiatives.
- Responsible for providing input to other functions in the organization and will likely be responsible for identification, prioritization, and definition of new processes. Participates as a member of the organization's executive leadership team and as staff support on committees of the Board of Directors, where designated.
- Ensures that resources are managed effectively, including alignment with Human Resources regarding the recruitment, t raining, development, motivation, and retention issues.
- Provides accurate forecasting and annual budget throughout the year to ensure that the budgetary goals are met or exceeded. Reviews productivity and utilization of the departments.
- Promotes and supports the implementation of the Affinia Healthcare vision and strategy, including defining goals and objectives for the clinics in line with the strategy.
- Evaluates team members, manages performance reviews, goal setting, and disciplinary action if required. Participates in formal staff review processes e.g., performance and salary review s to Affinia Healthcare's standards and timetables.
- Undertakes the full range of duties relevant to the leadership, management, and development of the staff to ensure their performance meets and or exceeds both the business and their own personal goals and objectives.
- Maintains a positive, results oriented work environment, building partnerships and modeling teamwork, communicating to the team in an open, balanced, and objective manner.
- Reviews and approves and assists to author clinical protocols and related documents.
- Performs other duties as assigned.
Marginal Functions
- Represents the organization at meetings with external attendees.
- Develops professional relations with opinion and thought leaders.
- Participates in marketing/business development activities as required.
- Participates as a member of Project Teams as needed.
