**Job Description:**
We are seeking a passionate Registered Nurse who is experienced in community health education and has a proven track record of delivering high-quality patient care. The successful candidate will have the opportunity to make a real difference in the lives of our patients and their families.
**Responsibilities:**
- Plan, organize, and direct hospice services with a focus on patient-centered care
- Develop and implement patient plans of care that meet individual needs and preferences
- Provide care utilizing infection control measures that protect both staff and patients
- Assure continuity of quality patient care through appropriate documentation
- Supervise and provide direction to home health aides and LPNs as needed
**Requirements:**
- Bachelor's degree in Nursing from an accredited institution
- Two years of experience as a Registered Nurse in a clinical care setting, home health or hospice preferred
- Nursing skills as defined by generally accepted standards of practice
- Proof of CPR, Hepatitis consent/declination, and Covid Vaccination or eligibility for a religious/medical exemption
