We are currently seeking a Registered Nurse Case Manager to join our rapidly growing team to oversee Westchester County. At Affirmed Home Care, we believe in compromising nothing when it comes to providing exceptional, client-centered care. This philosophy extends not only to our clients, but to our staff as well.
As the RN Case Manager, you will be responsible for developing and maintaining client-specific care plans, ensuring these plans are followed, and providing for the client's ongoing needs in a private pay, concierge level home care. This role requires 70% fieldwork for a small caseload of clients demanding great attention to detail.
The salary for this full-time position ranges from $110,000 to $130,000, commensurate with related experience.
QUALIFICATIONS :
- Current NYS RN license in good standing
- BSN preferred
- Minimum of two years recent clinical experience within a Home Care setting
- Strong assessment and organizational skills
- Strong time management skills
- Prior supervisory experience with field staff
- Proven track record of working independently
- Authorization to work in the US and up-to-date COVID vaccination required.
