We are expanding our program to off Evening IOP!!! We are looking for a motivated case manager to assist in implementing this option to our patients. Shifts will typically be 4 hours (5pm-9pm) 3-5 times per week.
Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a vital role in supporting individuals and families by providing guidance, resources, and advocacy. This position requires a strong understanding of behavioral health, crisis intervention, and workforce development. The Case Manager will work closely with clients to assess their needs, develop personalized plans, and facilitate access to necessary services. Facilitate group therapy sessions.
Responsibilities
- Conduct comprehensive assessments to identify client needs and strengths.
- Develop and implement individualized service plans tailored to each client's goals.
- Provide crisis intervention and support during critical situations.
- Utilize motivational interviewing techniques to encourage client engagement and progress.
- Mentor clients in developing life skills and achieving personal objectives.
- Facilitate access to community resources, including addiction counseling and childcare services.
- Collaborate with other professionals, including healthcare providers and social services, to ensure holistic support for clients.
- Monitor client progress and adjust service plans as necessary.
- Maintain accurate documentation of client interactions and progress in compliance with organizational policies.
- Facilitate group therapy sessions
Qualifications
- Experience in behavioral health or related fields is preferred.
- Knowledge of addiction counseling practices is a plus.
- Excellent crisis intervention skills with the ability to remain calm under pressure.
- Effective communication skills, both verbal and written.
- Ability to work collaboratively within a multidisciplinary team environment.
