Job Summary
A Client Care Coordinator in a home health care agency acts as a central point of contact for clients and their families, managing their care needs and ensuring a smooth, coordinated experience. They are responsible for client intake, assessment, scheduling, and communication with caregivers and other healthcare professionals.
Duties
- Client Intake and Assessment:Conducting initial and ongoing client assessments to determine their needs and develop individualized care plans.
- Service Coordination:Scheduling and coordinating home health services, including caregiver visits, therapy sessions, and other related care.
- Communication:Serving as a liaison between clients, families, caregivers, and other healthcare providers, facilitating clear and timely communication.
- Service Oversight:Monitoring the quality of care provided and addressing any issues or concerns that may arise.
- Documentation:Maintaining accurate and up-to-date client records, including assessments, care plans, and visit reports.
- Problem Solving:Identifying and resolving problems or challenges related to client care, such as scheduling conflicts or communication issues.
- Customer Service:Providing excellent customer service to clients and their families, ensuring their needs are met and their concerns are addresses
- Referral and Intake:Taking client referrals and conducting intake interviews to assess needs and determine eligibility for services.
Requirements
- Previous experience in a home health care setting is preferred or HCBS setting.
- Familiarity with medical administrative support functions is highly desirable.
- Strong understanding of medical terminology to effectively communicate with patients and healthcare professionals.
- Excellent organizational skills with the ability to manage multiple tasks efficiently.
- Proficient in using office software and equipment, including scheduling systems and electronic health records (EHR).
- Exceptional interpersonal skills to foster positive relationships with patients and team members.
- Ability to work in a fast-paced environment while maintaining attention to detail.
Job Types: Part-time, Contract, Temporary
Pay: $19.80 - $24.42 per hour
Schedule:
- Monday to Friday
- Weekends as needed
