Job Summary: We're seeking a compassionate Hospice and Palliative Nurse Practitioner to join our team. In this role, you'll provide high-quality care to patients and their families, addressing complex medical and emotional needs. You'll work collaboratively with a multidisciplinary team to develop and implement individualized care plans that prioritize patient-centered care.
Duties and Responsibilities:
- Assess patients' physical, emotional, and spiritual needs, developing individualized care plans to address these needs
- Provide direct patient care, including symptom management and pain control
- Collaborate with physicians and other healthcare professionals to develop and implement care plans
- Educate patients, families, and caregivers on disease management and end-of-life care options
Requirements:
- Master's degree in Nursing (MSN) from an accredited program
- Current licensure as a Registered Nurse (RN) in the state of practice
- 2 years of experience in hospice or palliative care
- Strong communication and interpersonal skills
