Job Description:
- Provide palliative care and hospice services to patients in their homes or other settings
- Collaborate with healthcare teams to develop and implement personalized care plans
- Educate patients and families on disease management and end-of-life care
Requirements
To be successful in this role, you will need:
- A graduate degree from an accredited nursing program
- At least 2 years of experience in chronic disease management or long-term care
- Hospice background preferred
- The ability to sit, stand, bend, move intermittently, and lift up to 100 pounds
