Job Summary
We are seeking a compassionate and skilled Nurse Practitioner to join our team in providing high-quality hospice and palliative care services. As a key member of our team, you will have the opportunity to make a real difference in the lives of our patients and their families.
Key Responsibilities
- Provide high-quality, patient-centered care to patients with serious illnesses.
- Collaborate with healthcare teams to develop and implement comprehensive care plans.
- Provide education and support to patients and their families on managing symptoms and improving quality of life.
- Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improving outcomes.
Requirements
- Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.
- 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred.
- Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively.
