About the Role
We are seeking a compassionate and dedicated Nurse Practitioner to join our team in providing high-quality hospice and palliative care services. As a key member of our team, you will work closely with patients and their families to alleviate symptoms and improve quality of life.
Responsibilities
- Collaborate with healthcare teams to develop and implement comprehensive care plans for seriously ill patients.
- Provide ongoing palliative care and hospice services to patients in their homes or in-patient facilities.
- Conduct thorough assessments of patient needs and create personalized care plans that address physical, emotional, and spiritual needs.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
Qualifications
- Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.
- 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred.
- Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively.
