The ideal candidate will have 2 years of experience within the last 5 years in chronic disease management or long-term care. A background in hospice is preferred but not required.

Responsibilities:
- Provide ongoing palliative care and hospice services
- Develop and implement patient care plans in collaboration with healthcare teams
- Provide education to patients and their families on end-of-life care and symptom management
- Participate in interdisciplinary team meetings to discuss patient care and coordinate services
Qualifications:
- Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing
- Ability to sit, stand, bend, move intermittently, and lift at least 80-100 lbs
