Qualifications:
- A heart to serve patients and families
- Education: Graduate of an accredited nursing program
- Experience: 2 years experience in chronic disease management/long-term care, hospice background preferred
- Reliable transportation, ability to sit, stand, bend, move intermittently and lift at least 25lbs
