This position is directly supervised by the Medical Director and performs services and supports that are based on a strength-based system of care treatment approach. The services are in accordance with being person centered, community based, building informal supports, and assisting consumers in developing skills to function independently. There's an emphasis on adapting services to ensure consumer and family involvement, self-determination in treatment planning, quality of care delivery, and making the community the primary focus of services and support.
Provides psychiatric evaluations to new clients and ongoing as needed.
Provides follow-up appointments to patients in accordance with expectations provided by the Medical Director and Clinical Director.
Develops diagnostic impression from review of Comprehensive Clinical Assessment, interview with client and family therapist, using a team approach.
Provides tele psychiatry services.
Ensures that services are delivered in a clinically appropriate manner and care provided is in compliance with North Carolina Board guidelines.
Evaluates the effectiveness of the program through reviews and evaluation of personal outcomes.
Participates in clinical learning forums (CLF's) with the Medical Director, Clinical Director, and others as appropriate, for sharing information and input about clinical issues affecting the program and to ensure the team is providing the best level of care to patients.
Participates in staffing for complex or high risk individuals.
Attends medical collaboration and supervision with the Medical Director, as decided upon between the FNP and Medical Director, to review difficult mental health cases, peer review issues, psychiatric consultation, CLF recommendations, etc.
Reviews and approves service orders (PCP's) for enhanced services (day treatment, residential level II or level III, intensive in-home, therapeutic foster care). When not in agreement with PCP and service recommendations, must collaborate with lead therapist and Clinical Director to assist in alternate plan and / or service.
Assures treatments delivered are current, appropriate, and follow accepted guidelines and community standards, and are not experimental in nature.
Completing paperwork and scanning in required documentation as required by APSM 45-2 guidelines and as requested by the medical records staff to ensure all records are accurate and billable.
Works with reimbursement staff to complete the service activity log (SAL) for accurate billing.
Orders, interprets, and acts upon laboratory tests, values, etc. as needed and recommended by drug regime.
Communicates openly and honestly with all members of the Focus team and adhere to team dynamics.
Report critical incidents with local and state agencies and the Quality Improvement Director.
Prepares and modifies documents including correspondence, reports, drafts, memos, and emails pertaining to client and / or assigned caseloads.
Ensures an integration of care approach for individuals with psychiatric illnesses in the accepted standard of clinical care.
Assists agency staff with making referrals within the agency or outside the agency for needed specialty services. Assist staff with linkages to other providers for appropriate services not provided by the CABHA agency.
Assists with CARF certification as it relates to the medication management service area.
In collaboration with the Administrative Director and QI Director, assists in the review of policies, medication management forms (ensuring excellent care and services), and for accuracy and updating.
Participates in quality improvement programs including health and safety.
Responsible for attending to all the rules and regulations of the funding sources (DMA, IPRS, private insurance, etc.), agency, state and federal licensure, and the rules of quality improvement, medical records, finance, etc. of the agency.
