About the Role:
We are seeking a skilled Nurse Practitioner to join our team. As a Hospice and Palliative Care Nurse Practitioner, you will care for seriously ill patients, alleviating burdensome symptoms and aligning medical treatments with their goals and values. You will collaborate with a team of like-minded clinicians and teammates to deliver high-quality care wherever patients call home.
Responsibilities:
- Provide ongoing palliative care and hospice services
- Educate patients, families, and caregivers on disease management and end-of-life care options
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
- Conduct assessments, develop care plans, and provide direct patient care
Qualifications:
- Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing preferred
- 2 years experience within the last 5 years in chronic disease management/long-term care
- Hospice background preferred
- Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively
