Job Summary:
We are seeking a skilled Nurse Practitioner to join our palliative care team. The successful candidate will have expertise in assessing patients with advanced illnesses, developing individualized care plans, and collaborating with physicians and other healthcare professionals to coordinate care.

Key Responsibilities:
As a Nurse Practitioner, your key responsibilities will include:
  1. Conducting comprehensive assessments of patients with advanced illnesses to identify their physical, emotional, and spiritual needs.
  2. Developing individualized care plans that address the patient's needs and preferences.
  3. Collaborating with physicians and other healthcare professionals to coordinate care and ensure seamless transitions between settings.
  4. Providing education and support to patients and their families to empower them to make informed decisions about their care.

Requirements:
To be successful in this role, you will need:
  1. A master's degree in nursing or a related field.
  2. A minimum of two years of experience as a nurse practitioner, preferably in palliative care.
  3. Strong leadership and communication skills, with the ability to work effectively in a fast-paced environment.
  4. Certification in hospice and palliative care (ACHPN) preferred.
