- Highly organized and detail oriented
- Excellent written and oral communication skills
- Two years of experience as a DON in a Home Health setting
- Knowledge of CMS star ratings and how process and outcome measures impact data
- Ability to review and utilize quality assurance programs and protocols to identify and resolve issues
- Strong history of compliance with all regulations and clinical system
- Thorough understanding of the issues related to the delivery of home healthcare services
- Strong follow-up, problem-solving, and critical-thinking skills
Basic job responsibilities will include but are not limited to the following:
- Quality assurance including reviewing charts and notes
- Staff education for OASIS-E
- Oasis knowledge to include hands-on experience with review
- Manages the quality and timeliness of daily functions through scheduling, monitoring attendance, and supervising assigned staff
- Knowledge of leading practice in clinical care and payor requirements
- Working knowledge of managed care and the home health continuum, as well as utilization management and case management
- Working knowledge of the concepts associated with Performance Improvement
- Plans, organizes, and directs overall clinical operations to provide comprehensive and excellent patient care in accordance with federal, state, and local guidelines
- Develops and implements nursing service objectives, processes, policies, and procedures to maintain superb patient care and compliance
- Must be able to manage nursing care in the skilled home health and state-funded long term care programs.
- Must be able to perform skilled assessments/visits under skilled and long-term care departments as needed. Provide on-call coverage as needed
Requirements:
- Must be an RN licensed in the state of Texas
- Bachelor's Degree in Nursing preferred
- Must have two years of DON experience in a Home Health setting
