Job Summary :
The Team Manager (TM) works in collaboration with the Director Clinical Operations (DCO), Executive Director of Clinical Operations (EDCO) or Regional Vice President of Clinical Operations (RVPCO) as well as the Interdisciplinary Group (IDG) to ensure that care and services provided by the team are delivered in a fiscally responsible manner while maintaining strict compliance with all federal, state, and local rules and regulations. The TM, in collaboration with the DCO, EDCO or RVPCO and IDG, will assist in ensuring that appropriate and quality services are provided through the continuum of care.

MAJOR JOB FUNCTIONS :
Promote and practice the Sage Family of Companies philosophy and mission of “Providing the most compassionate, comprehensive, and qualified care to people with a life-limiting illness to serve them through their individual journey with dignity, respect, and integrity.”Assumes co-ownership along with the DCO, EDCO or RVPCO in responsibility for maintaining quality hospice patient care while following the Company's objectives and policies through planning, coordinating, implementing, and evaluating the services provided to patients, families, and customers.Assists the DCO, EDCO or RVPCO in the coordination of clinical care and collaborates with members of the IDG.Works with the DCO, EDCO or RVPCO to ensure operational indicators such as per patient day costs are within budget related to medical supplies, durable medical equipment (DME), and mileage.Receives and processes patient referrals, as needed - may perform payor verification and assist with obtaining appropriate documents to ensure a complete and timely admission.Coordinates with the DCO, EDCO or RVPCO all team activities, patient assignments, staffing patterns, and schedules to ensure patient and family needs are met in a timely and cost-effective manner.Ensures workflow tasks are completed promptly within the electronic medical record (EMR) system.Ensures all outstanding physician orders are returned promptly, signed, dated, and uploaded to the EMR.Ensures all processes around patient care are followed to maintain compliance with federal, state, and accrediting body regulations.Ensures effective communication with all team members when there is a new admission, a change in the plan of care (POC), or a change in level of care (LOC) for a new or existing patient.Educates patients and families or caregivers on, and assists in, preparation of advance directives.Provides information and referral services for patients and families or caregivers regarding practical and environmental needs.Provides information to patients and families / caregivers and community agencies.Actively participates in quality assessment performance improvement teams and activities.Verifies release of information and requests medical records as appropriate – once received, is responsible for scanning the records into the EMR and notifying the DCO, EDCO or RVPCO.Contacts other agencies to collaborate transfers as well as patients leaving the service area under a traveling contract.Works in collaboration with the DCO, EDCO or RVPCO to ensure that all Face-to-Face visits are scheduled timely, and the providers are notified promptly to ensure regulatory compliance – tracks Face-to-Face documentation and ensures documents are accurate and completed on approved form before uploading to the medical record.Processes (sends and tracks completion) of changes in the patient record : Deaths, discharge, revocation, Location, level of care, next of kin, contact information, personal, and any other information requested.Ensures that appropriate and thorough preparation is completed for the IDG meeting to ensure a compliant and timely meeting.Collaborates with IDG members to ensure that all IDG notes are completed promptly, but no later than the end of each IDG meeting.Acts as scribe in IDG meetings, as needed.Runs reports in Home Care Home Base, as scheduled, to ensure timely follow-up on missing items, including but not limited to, scheduling items, unverified visits, or any other bill hold items that may need attention.Performs other duties as assigned.REQUIRED EDUCATION / EXPERIENCE :
Licensed Practical Nurse or two (2) years of experience in a healthcare setting.Must have reliable transportation, proof of current automobile insurance and a valid driver’s license.SKILLS / ABILITIES / KNOWLEDGE :
Experience using an electronic medical record (EMR) system.Familiarity with healthcare regulations.Complies with acceptable professional standards and practice.Successfully navigate the Microsoft suite of software.Ability to work with standard office equipment (copier, fax, scanner, etc.).Demonstrate excellent communication skills.Be team oriented and self-motivating.Because those who care deserve more.
