Depending on your role and your hours, we offer
Position Responsibilities : Care Managers are responsible for the ongoing management of a caseload of State Home Care consumers including conducting in-home assessments to assist them to obtain services and programs which will enhance their quality of life and allow them to live as independently as possible with dignity and respect.

Qualifications : Bachelors degree in social work, human services or a related field required. Associates degree with significant relevant work experience may be substituted for a portion of degree. Experience working with elders strongly preferred. Strong written, verbal and computer skills required. Valid Driver's License and reliable transportation required as field work is an essential component of the position.
Hours : 37.5 per week
