HOW YOU'LL MAKE A DIFFERENCE IN THIS ROLE (ESSENTIAL FUNCTIONS)
The Patient Care Coordinator Lead is responsible for scheduling Homecare and Resource staff and providers. This role acts as a resource to the team, providing training and mentoring and efficiencies.
Scheduling
- Complete all Homecare and Resource discipline scheduling (e.g. RN, SW, CNA, etc.) across all shifts.
- Coordinate staffing to meet patient needs as a result of staff call-ins / absences.
- Assign patient visits to clinical staff ensuring consistency and following established scheduling best practices.
- Maintains staffing matrix, PTO spreadsheet and approvals for all Homecare and Resource discipline scheduling (e.g. RN, SW, CNA, etc.).
- Manages the On Call schedules for the Home Care and Resource staff.
Quality and Service
Monitor daily activities of Patient Coordinators and act as resource
