HOW YOU'LL MAKE A DIFFERENCE IN THIS ROLE (ESSENTIAL FUNCTIONS)

The Patient Care Coordinator Lead is responsible for scheduling Homecare and Resource staff and providers. This role acts as a resource to the team, providing training and mentoring and efficiencies.

Scheduling

Complete all Homecare and Resource discipline scheduling (e.g. RN, SW, CNA, etc.) across all shifts.
Coordinate staffing to meet patient needs as a result of staff call-ins / absences.
Assign patient visits to clinical staff ensuring consistency and following established scheduling best practices.
Maintains staffing matrix, PTO spreadsheet and approvals for all Homecare and Resource discipline scheduling (e.g. RN, SW, CNA, etc.).
Manages the On Call schedules for the Home Care and Resource staff.

Quality and Service

Ensure timely assignments of new admissions as well as accurate and timely scheduling of existing patients requiring C.N.A., RN and / or SW support.

Report patient and staffing concerns to Manager.

Problem solves immediate visit scheduling needs.

Identify department efficiencies and implements process improvements.

Monitor daily activities of Patient Coordinators and act as resource

Provide support to team members in problem-solving and answers questions as able, referring up to supervisor as necessary.

Mentor new employees and provide ongoing team training.

Monitor Patient Care Coordinators for consistent quality standards. Identify concerns and report them to management.

Assist with evaluation and testing of process changes and new technology.

In partnership with managers, provide timely recognition and coaching and provide input for performance discussions.

Other Duties

Participate in internal work groups, meetings and committees relative to position responsibilities.

Ensure compliance with Agrace policies and protocols, state, federal, and other regulations.

Qualifications

YOU BRING (EXPERIENCE & QUALIFICATIONS)

High school diploma or GED or equivalent combination of education and work experience, required

Two or more years scheduling experience, preferably in complex healthcare or hospice environment

Ability to work weekend rotation.

Ability to maintain professional boundaries with patients, families and co-workers

Intermediate Microsoft Office skills

Intermediate electronic health records skills; Epic preferred

Ability to work effectively and cooperatively with others

Ability to communicate effectively verbally and written

Ability to be flexible in time and task to meet organizational needs

Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property

Ability to use proper body mechanics, safe patient handling, and standard precautions
