**Job Description**:
We are seeking a Qualified Professional (QP) specializing in developmental disabilities to join our team. The QP will be responsible for overseeing the development, implementation, and evaluation of care plans for individuals with Caregiver services, providing support and supervision to direct care staff. This position plays a critical role in helping individuals achieve their personal goals and improve their quality of life.
**Key Responsibilities**:
- Develop and oversee individualized care plans for individuals with services
- Supervise and train direct care staff, ensuring they are competent to deliver services.
- Evaluate the effectiveness of services and supports, making adjustments as necessary.
- Coordinate services based on the individual’s needs and goals, ensuring compliance with all applicable regulations.
- Monitor and assess staff performance in implementing positive behavior support programs.
- Document service delivery and ensure adherence to service plans.
- Provide on-the-job training for staff, particularly in areas related to developmental disabilities.
**Qualifications**:
- Bachelor’s degree in a field related to human services, education, or health, with one year of full-time experience providing direct care services to individuals with disabilities or older adults.

- Associate’s degree in a related field, with two years of full-time experience providing direct care services to individuals with disabilities or older adults.
*OR*
- Diploma from an accredited institution in a related field, with three years of full-time experience providing direct care services.
*OR*
- A minimum of 50 hours of education and training in human services and disabilities, plus four years of full-time work experience providing direct care services under the supervision of a qualified professional.
