Social Worker (FT, Days) Monterey Park Hospital

Overview :

Monterey Park Hospital, located in San Gabriel Valley of Los Angeles County, is recruiting for a full time Social Worker for our Utilization Review Department. Monterey Park Hospital is a 101 bed acute care community hospital providing our community with advanced medical and culturally sensitive health-care services for over 50 years. The Clinical Social Worker reports to the Chief Operating Officer.

Responsibilities :

The Social Worker provides direct social work services and counseling to patients and their families, provides psychosocial and diagnostic impressions on all patients seen, evaluates social / family information and determines plan for social treatment. Consults with physicians and health care personnel about patients or situations which directly impact patient's illness / hospitalization, maintains a continuity of care by following up with patients referred for Social Service Intervention, assists in the provision of crisis casework to Emergency Dept. patients referred by that department for assessment, counseling and referral. Coordinates family conference in which patient's goals and progress may be discussed. Maintains close working relationship with all appropriate community agencies. Participates in hospital inservice programs regarding the social and emotional components or illness and their effects on patients / families. Performs basic management tasks related to job such as time planning, recording, reports, statistics, and other routines as required by the Utilization Review Department. Attends appropriate department and hospital inservices, participates in professional organizations and activities outside as they relate to the position and assists in departmental studies and projects as assigned by the Director.

Qualifications :

Minimum Education : LCSW or MSW degree required.

Minimum Experience : 2-3 years of acute care experience in a hospital setting.
