RN Home Health Clinical Lead

Responsibilities

In this position, you will be responsible for:

1. Conducting thorough assessments of patients' medical and social needs
2. Developing and implementing individualized care plans in collaboration with other healthcare professionals
3. Providing education and support to patients and their families
4. Documenting patient information accurately and efficiently
