Initiate or modify medication therapy care plans on the basis of patient responses.
Expanded Functions
Includes prescriptive authority where this is legally and organizationally authorized such as PrEP and PrEP therapies.

Essential Duties And Responsibilities
- The Pharmacy Director is expected to maintain, implement and practice the following, in addition to his or her administrative responsibilities.
- AHF Ambulatory Care Pharmacist Practitioner’s Essential Functions
- Perform patient assessment for medication-related factors.
- Interpret data related to medication safety and effectiveness.
- Perform medication reconciliation of patients’ therapy.
- Provide information, education, and counseling to patients about medication-related care.
- Document the care provided in patients’ records.
- Identify any barriers to patient treatment and compliance.
- Participate in multidisciplinary reviews of patients’ progress.
- Communicate with payers to resolve issues that may impede access to medication therapies.
- Communicate relevant issues to physicians and other team members.
- Medication Therapy and Primary Care Collaborative Practice Agreements (where appropriate).
Initiate or modify medication therapy care plans on the basis of patient responses.

Education, Certificates, License, Registrations, And Experience
- A Pharm. D degree or higher desirable.
- Current Pharmacist license in the state employed by AHF.
- HIV certification highly desirable.
- National HIV certification highly desirable.
- Must have at least three years (3) recent experience in a supervisory role.
- Experience managing multiple sites highly desirable.
- Experience within AHF Pharmacy is desirable and favorable.
- Must have typing skills and familiarity with Windows PC applications including word processing, excel spread sheets and various e-mail and texting applications.
- Experience with data input and experience with a Pharmacy Management System (PMS) or Point of Sale System (POS), is highly desirable.
- Be able to learn the use of various computer software programs utilized by AHF throughout the organization, where appropriate.
- Must have the ability to read and interpret various types of prescription documents.
- Must learn to retrieve patient documents and correspondence from various electronic communication devices such as e-mail, text messaging, etc.
- Must have the ability to read and interpret policies, procedures and guidelines.
- Must have the ability to read and interpret documents such as safety rules, operating and maintenance instructions. Next
- Must have the ability to write routine reports and correspondence, including entries in patient’s profiles in the PMS.
- Bilingual in English and Spanish is highly desirable.
- Must have at least intermediate skills to calculate figures and amounts such as a day’s supply of medications based on the prescription directions.
- Must have the ability to calculate the monthly supply of medication required based on the instructions of a prescription.
- Must have the ability to calculate dispensing and billing cycles based on days’ supply and billing dates.
- Must have the ability to calculate figures and amounts such as discounts, volume, percentages related to prescription charges and selling prices of goods and services provided by AHF Pharmacy.
- Is responsible for the overall direction, coordination, and evaluation of assigned employees.
- Carries out supervisory responsibilities in accordance with AHF’s policies and applicable laws.
