AIM Behavioral Health (Ashland Integrative Medicine) in Ashland, Kentucky is an Addiction Treatment Medical Practice – specializing in evidence-based substance abuse treatment. We are currently seeking an experienced, self-motivated INDEPENDENT CONTRACT CASE MANAGER to become part of our Behavioral Health Care Team. The Case Manager is responsible for providing direct patient care and helps support the success of their patients through positive collaborations with our physicians, clinical staff, and other providers in the community. You will design strategic care plans to assist the patient in recovery through Targeted Case Management.
Duties
As follows, and otherwise as mutually agreed upon.
Orient all patients to our program and explain what services and resources are available, explain assignments and due dates so patients fully understand our program requirements.
Maintain confidentiality of records relating to patients and their treatment.
Responsible for submitting all prior authorizations prior to rendering services.
Responsible for making sure patients are compliant in clinic attendance and attempting to reaching their goals.
Must be certified as a TCM in Kentucky or complete state approved TCM training within 6 months of employment.
Manage patient referrals to PCP, specialist and or community resources as needed.
Develop and assist after care plan with patients when it is deemed appropriate by attending physician.
Maintains rapport and active working relationship with other medical practices, physicians, governmental agencies, churches, Non-Profit Agencies, and others in the community.
Requirements
- Bachelor's Degree in a Health Science or Human Services Field (psychology, sociology, social work, family studies, human services, counseling, nursing, or another human service degree program related to working with individuals with substance use disorders).
- MINIMUM 2 years of full-time employment working in a Human Services setting OR Master's Degree in Behavioral Science, as defined above. (Addiction Services experience given preference)

LEADERSHIP – the individual inspires and motivates others to perform well and accepts feedback from others.
DEPENDABILITY - Performs contracted work that is agreed upon in line with agreement and applicable regulations.
QUANTITY AND QUALITY OF WORK – Completes work with a high level of accuracy and a low level of errors. Produces neat, thorough and organized work.
ATTITUDE – Demonstrates a cooperative, positive, and enthusiastic attitude towards Aim Behavioral team, patients, and visitors. Knows and follows confidentiality laws and procedures at all times. Is effective in written and oral expression. Demonstrates proper telephone etiquette and able to communicate easily. Conducts themselves in manner consistent with the agency’s code of ethics and code of conduct.
TEAM WORK / COLLABORATION - Direct, honest, tactful and respects the contribution of others. Interacts and cooperates with others to ensure the agency’s objectives and goals are met. Demonstrates flexibility in responding to priorities and organizational change. .
