Job Overview
We are seeking a dedicated and compassionate Case Manager to join our healthcare team. Responsibilities
- Conduct comprehensive patient assessments to identify needs and develop individualized care plans.
- Coordinate discharge planning for patients transitioning from acute care settings, ensuring continuity of care.
- Collaborate with multidisciplinary teams to facilitate effective communication and resource allocation.
- Provide addiction counseling and support for patients dealing with substance use disorders.
- Monitor patient progress and adjust care plans as necessary to meet evolving needs.
- Engage in utilization management activities to optimize resource use and improve patient outcomes.
- Advocate for patients by connecting them with community resources and services that promote recovery and well-being.
- Maintain accurate documentation of case management activities in compliance with regulatory standards.
Qualifications
- Bachelor’s degree in Social Work, Nursing, or a related field; Master’s degree preferred.
- Proven experience in case management, particularly in acute care or hospital medicine settings.
- Familiarity with discharge planning processes and utilization management principles.
- Strong skills in patient assessment and the ability to develop comprehensive care plans.
- Experience working within Pediatric Intensive Care Units (PICU) is a plus.
- Knowledge of outpatient services and community resources available for patients.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work collaboratively within a multidisciplinary team environment while advocating for patients' needs.
