Full Job Description
As a Provider, you will interview with families to find the perfect match! Once paired with your client, you will have the opportunity to become certified (if not already) to perform any of the following services your client has:
Habilitation Working on individual goals your client has which might include self-help skills, attention to task, communication, etc.
Respite Taking care of your client while the primary caregiver takes a break. This can include outings, movies at home, or anything your client enjoys!
Attendant Care Helping your client with personal care activities. This might be brushing teeth, preparing meals, doing laundry, etc...
All services require CPR, First Aid, Article IX, and a Class 1 Fingerprint Clearance Card. Don't worry, we can help you and prepare you to best service your client!

Basic & Key Responsibilities:
- Dress Residents
- Feed Residents
- Assisting with Basic Household Chores
- Coordinating Grocery Deliveries
- Transport Residents to Appointments, Visits, and Activities
- Provide emotional support & companionship
- Monitor and ensure the health and safety of all individuals living in the home.
- Administer medications as prescribed by a medical professional.
- Prepare all necessary meals (breakfast, lunch, dinner, snacks etc.).
- Implement habilitation objectives/goals as agreed upon by the ISP team.
- When applicable implement behavior building plans.
- Actively interact with individuals in the home throughout your shift.
- Complete all required documentation before leaving shift.
- Assist with preparing monthly activity calendar and weekly menus.
- Ensure the home is clean and in good repair.Duties as assigned.
As a Caregiver / Direct Care Professional
- You are a mandatory reporter. If you suspect any type of abuse or neglect; you will contact the appropriate authorities, DCS/APS, and your direct supervisor.
- Completion of Incident Reports as directed.
- Maintaining current qualifications and trainings.
Qualifications:
- Ability to obtain/maintain a Level One Fingerprint Clearance Card
- Ability to obtain/maintain a valid Drivers License
- Must be at least 18 years of age
- Proficient with the English language: written and verbal
Duties
- Reading, writing, and carrying out directions
- Charting or taking notes for others to review
- Providing companionship and supportive listening
- Reporting changes in the health or status of the client
- Taking vitals, height, and weight
- Feeding clients
- Maintaining patient’s health equipment
- Responding to emergency situations
- Administering medication
- Transporting client to appointments
- Facilitating community and recreational outings, including transport
- Providing support with hobbies, studies, and social activities
