Job Description: As a Care Coordinator, you will play a pivotal role in:
- Building meaningful relationships through onsite visits to members' homes, understanding their unique needs and preferences.
- Implementing Person-Centered Service Plans (PCSP) and championing positive changes in PCSP meetings.
- Conducting interviews for Direct Support Professionals, ensuring a perfect match based on individual needs.
- Monitoring service quality, offering guidance and support to enhance the client and provider experience.
- Managing the reciprocal relationship between the provider, client, care team, and the organization for mutual success.
- Educating prospective DDD members on the invaluable benefits of HCBS services.
- Participating in company events, meetings, and maintaining strong community partner relationships.
- Staying compliant with state DDD & AHCCCS policies through regular review and attendance at relevant meetings.
Requirements:
- Prior experience in roles such as HCBS Coordinator, Care Manager, Group Home Manager, or Direct Support Professional.
- A genuine passion for the special needs community and a commitment to making a positive impact.
- Knowledge of Developmental Disabilities programs and service systems in Arizona.
- Proficient skills in Case Management (Interviewing, Advocacy, Collaboration).
- Ability to maintain confidentiality with knowledge of HIPAA.
- Proficiency in computer skills, with familiarity using Apple products & Google Drive.
- Valid Level One fingerprint clearance card.
- In-state travel required; must possess a valid state-issued driver's license and access to personal transportation (mileage reimbursed).
- Subject to a search of both the Child Protective Services Central Registry and the Adult Protective Services Registry.
