Aldersbridge Communities is hiring a full-time Memory Care Program Director for The Loft at Linn Memory Care Assisted Living facility. Our Memory Care Unit consists of 44 apartments. Working for the Memory Care Unit will foster your commitment to enriching the lives of our residents.


This position promotes and integrates the mission and philosophy of Aldersbridge Communities. The Memory Care Program Director is responsible for the day-to-day operations embracing the household model in the Memory Care Program. The Memory Care Program Director is responsible for overseeing compliance with all community policies and procedures and applicable state and federal regulations as they apply to the Memory Care Program. The Memory Care Program Director will create a supportive environment with focus on resident directed living, providing purpose, and meaning for each resident and providing autonomy in decisions of every aspect of care and support we provide.


Education/Experience

Education:

Bachelor’s degree in social work, Gerontology, Psychology, Recreational Therapy or Counseling preferred. Five to seven years’ experience in health, human services, and/or community services (service coordination, case management with the elderly population (or related field).

Experience:

At a minimum 2 years supervisory experience necessary. Must have a demonstrated ability to hire, direct, and manage personnel. Demonstrated working knowledge of assisted living, supportive services in housing environments, long-term care, as well as resources for elderly population. Knowledge of HUD regulations; federal and state regulations and guidelines relative to housing, assisted living, and community-based service programs. Knowledge of Microsoft Office computer applications.


Responsibilities:

  • Conducts interviews, orientation, and ongoing training of Memory Care Program staff in accordance with staff training policy and regulatory guidelines in collaboration with Wellness Director.
  • Embraces the philosophy of the Memory Care Program, in all aspects of his/her role to include: interactions with resident, family, and staff and the daily oversight of the Memory Care Program’s operations in collaboration with Wellness Director and Administrator.
  • Manages all Memory Care Program staff in partnership with the Wellness Director(s) or designee.
  • Plans, coordinates, and evaluates the Memory Care Program’s resident programs.
  • Assist with scheduling and conducts evaluations of all Memory Care Program’s Staff in collaboration of Wellness Director.
  • Hires, trains, supervises, motivates, and evaluates staff to ensure the provision of high-quality services to residents in a manner consistent with the philosophy of the organization and its programs.
  • Develop a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified.
  • Ensure that all residential services personnel are aware of the care plan and that care plans are used in providing daily social services to the resident.
  • Assist in preparing and planning the residential services department’s budget for equipment, supplies, and labor and submit to the Administrator for review, recommendations, and approval.
