What You'll Do
Under direct supervision of the Director of Access Services, the transition coordinator is responsible for removing barrier, providing support to the following programs: MST, IIH, PRTF (scanning only,) Day Treatment (scanning only) as needed and ensuring clients have access to services within a reasonable timeframe.

Qualifications
Education and Experience
- Bachelor’s Degree required
- Must be QMHP as verified by AYN
- Behavioral health front desk, medical office experience preferred.
- Proficiency in MS Office applications
- Valid Driver's License
