What are the responsibilities and job description for the Program Supervisor – Patient Navigator Services position at Alivio Medical Center?
SUMMARY:
The Program Supervisor coordinate Alivio Medical Center’s staffing services at Cook County Health (CCH) for patient education, medical care navigation and retention services to improve maternal and child health outcomes and patient satisfaction for CCH patients. The Navigation services include onboarding and retaining CCH obstetrical patients and other women into early and adequate prenatal care, reducing the percentage of low birth weight and preterm infants, and linking participants to the network of services they need.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Supports the goals and philosophy of Alivio Medical Center
- Maintains adherence to all hospital and department specific safety requirements, as well as required service functions within a healthcare environment.
- Supervises Patient Navigators at all CCH locations.
- Plans, prepares, and assigns work schedules according to patient volumes and service demands, ensuring appropriate coverage at all assigned CCH locations.
- Responsible for ensuring employee onboarding/training is complete and identifies training opportunities to support staff.
- Conducts regular and timely performance reviews of staff, including one-on-one meetings to clarify results, encourage development and develop performance improvement plans. Provide coaching and feedback to enhance their performance.
- Conduct regular team meetings to communicate updates and set goals.
- Outline work plan to provide services throughout the Cook County Health provider network, experience with a culturally and linguistically diverse population.
- Participate in the development of materials that will help CCH patients with adherence to care, understanding care plans and navigating the CCH healthcare system.
- Submit monthly reports in appropriate format. Provides Performance and Reporting requirements including, but not limited to: program productivity metrics, number of patients onboarded to CCH as prenatal care medical home, expected delivery volume and actual delivery volume, distribution metrics of tangible resources to patients, metrics of hospital labor and delivery tours with improved health literacy, expected prenatal care visit volume among those enrolled into prenatal care, metrics for patient demographics served by navigators, and for tangible SDOH resources distributed.
- Maintains and reports program’s inventory.
- Monitors patient feedback and communicates to leadership accordingly.
- Support the Women and Children Service line in various CCH locations to support navigator staff needs.
- Meets regularly with Program Manager and communicates program progress and deliverables.
- Perform other related duties assigned.

WORKING CONDITIONS:
OSHA Category 3 – Involves no regular exposure to blood, body fluids, or tissues, and tasks that involve exposure to blood, body fluids, or tissues are not a condition of employment.
QUALIFICATIONS:
Education:
- Bachelor's degree or the equivalent of four years of college in community or public health or medicine field.
Certification or Licensure:
- N/A
Experience:
- 3 years of program management experience and supervisory experience.
- 3 years of experience in hospital work, desired, preferably in Maternal and Child Health.
Special Training:
- Understands and/or has had an opportunity to work on equity issues in healthcare, social services.
Demonstrated Competencies:
- Strong planning and implementation skills, including problem solving and troubleshooting skills.
- Ability to work both independently and as part of multi-organizational teams.
- Ability to drive to and supervise services at CCH Health Centers and Hospitals.
- Proficiency with Microsoft Office and electronic health record software.
- Excellent verbal and written communication skills in English and Spanish with exceptional attention to details.
- Demonstrated success developing and evaluating program models and selecting and successfully operationalizing innovative programs.
- Proficient in using technology as a management-reporting tool and experience working with statistics.

Salary range:
- $50,000- $55,000 Based on qualifications and experience.
