Patient Information Coordinator/Face to Face - Full time on site in Lynn office (Mon-Fri 8 am-4 pm)
ESSENTIAL DUTIES AND RESPONSIBILITIES:
PATIENT INFORMATION COORDINATOR DUTIES
· Perform electronic admission and discharge audit of patient records and follow up to obtain missing information.
· Process and send out medical records requests according to procedure.
· Process all requests for records for Insurance Audits (ADR’s)
· Send out and track electronic signature requests.
· Upload to Patients EMR, all documents received via fax, email, or mail.
FACE TO FACE SUPPORT DUTIES
- Review all Face to Face that arrive post referral. Create non-interim order and follow the process. Follow up if an interim order is necessary. Mark performed all F2F that are valid and follow-up on all F2F that are invalid.
- Maintain the F2F log in Document Manager. Utilize the log as a tracking device for ensuring that all F2F are followed up on regularly.
- Communicate with the Clinical team to ensure that an MD appointment is set up if a progress note is not available with referral and an MD appointment has not yet been set up.
- Run F2F events report to follow up on any discrepancies between Document Manager and Admissions that require F2F.
ADDITIONAL DUTIES AND RESPONSIBILITIES
· Participate in the reception coverage rotation as assigned by supervisor.
· Back up to other departments within operations when needed.
· Any other task assigned by supervisor.
QUALIFICATIONS
Experience, Education and Licensure
High School diploma or GED. Completion of course in medical records terminology preferred. Must be familiar with MS Word, Outlook, and Excel. Unless otherwise agreed upon by employer, employee must have a valid drivers license. Furthermore, employee shall have use of a vehicle during working times and insurance as a driver of that automobile.
Language Skills
Ability to read and write basic sentences and to converse intelligently with staff.
