We are looking for an experienced Care Management Manager to join our team in Fullerton, California. This full-time role will lead a team of Registered Nurses (RNs) and Social Workers (MSWs) to support patients across the care continuum.
Job Responsibilities:
Manage daily operations for the Care Management team.
Supervise Case Managers and Care Management Coordinators.
Help patients access care and use resources effectively.
Work with leadership to improve care coordination and outcomes.
Requirements:
Bachelor’s degree in Nursing or Master’s in Social Work.
At least 3 years of experience in an acute or ambulatory care setting.
Strong project management and operations skills.
Preferred:
Master’s degree in Nursing or Health Administration.
Case Management Certification (CCM or ACM).
3 years of management experience in healthcare.
