About the Position
The Case Manager I plays a crucial role in supporting the physician and interdisciplinary team in delivering high-quality patient care. This position aims to enhance clinical outcomes, patient satisfaction, and cost management while ensuring timely and accurate information is provided to payors.
Key Responsibilities:
- Facilitate collaborative patient care across the continuum, addressing barriers to efficient care delivery and reimbursement as needed.
- Apply process improvement methodologies to evaluate care outcomes.
- Ensure adherence to RN scope of practice, current state requirements, CMS Conditions of Participation, EMTALA, The Patient Bill of Rights, AB1203, and other relevant federal or state regulatory agency requirements.
Requirements:
- Bachelor's degree in nursing (preferred) or Associate's degree in nursing (required).
- Minimum one to three years of hospital or related experience.
- Able to navigate outside provider networks (Health Plans, IPA's, FQHC's) successfully.
- Bilingual language skills (Spanish) preferred.
- Basic computer skills.
- Current California Nursing license.
