Job Description
The Support Specialist will be responsible for providing support and guidance to our clients and their families. This includes working closely with clients to understand their needs and preferences and collaborating with service providers to ensure that care services are delivered in a timely and effective manner. The Support Specialist will also monitor and evaluate the effectiveness of care services and make adjustments as needed.
Responsibilities
* Provide support and guidance to clients and their families* Work closely with clients to understand their needs and preferences and ensure that they are respected and addressed* Collaborate with service providers to ensure that care services are delivered in a timely and effective manner* Monitor and evaluate the effectiveness of care services and make adjustments as needed* Ensure that all relevant documentation is accurate and up-to-date* Communicate regularly with clients and their families to keep them informed of progress and changes* Participate in ongoing training and education to enhance knowledge and skills* Adhere to agency policies and procedures* Contribute to a positive and supportive team environment* Perform other duties as assigned by supervisor or designee
Requirements
* Bachelor's degree in human services or related field* 2 years of experience in case management or related field* Licensed social worker or counselor in West Virginia* Strong communication and interpersonal skills* Ability to work independently and as part of a team* Proficiency in MS Word, Excel, and other software related to job functions
