What are the responsibilities and job description for the Veteran and Family Services Case Manager position at Alleghany Highlands Community Services?
Provides direct and indirect case management services for Service Members Veterans and Families (SMVF) enrolled in the Alleghany Highlands and Blue Ridge Behavioral Health catchment areas. Coordinates services and/or resources needed by veterans and family members to support their basic needs and enable them to remain and function optimally in the community. Provides education and support to family members to assist with the veteran's functioning. Identifies and refers SMVF seeking services to internal providers that have been trained in military cultural competency (MCC) and who specialize in treatment for Post-Traumatic Stress Disorder and other forms of trauma (including from military and/or combat service including Military Sexual Trauma) and Substance Use Disorders. The Veteran and Family Services Case Manager will also collaborate with Military Treatment Facilities (MTFs), Veterans Health Administration (VHA) facilities, Virginia Department of Veterans Services (DVS) programs and other external providers to determine SMVF eligibility for services. The Veteran and Family Services case manager ensures that Service Members Veterans and Families (SMVF) receive high quality, evidence-based, trauma-informed, culturally-competent, and accessible services. |
Minimum Requirements |
Must complete all AHCS required trainings for the position and maintain established certifications and trainings throughout employment. Knowledge, Skills and Abilities: Requires demonstrated knowledge of case management/social work practices and resources available to help individuals with mental illnesses and/or substance abuse disorders and their families/care givers in the community. Knowledge of military life and veterans services preferred. Basic knowledge of human rights, ethical and legal concerns as they relate to providing case management, clinical, residential, crisis stabilization services Demonstrated ability to communicate clearly and concisely (written and oral) and basic working knowledge of computers and word processing |
Education: Bachelor's Degree in Human Services field (required) |
Experience: 1 year previous experience in a setting working with individuals in need of case management or care coordination |
Essential Responsibilities and Function:
- Maintains current and comprehensive assessments of needs and strengths for veterans/family members. Monitors satisfaction with services, assuring that the proper documentation of services is maintained in the chart. Responds to new referrals within three business days.
- Refer SMVF served by the CSB to Dept. of Veterans Services (DVS), Veterans Health Administration facilities and services (VHA), and/or Military Treatment Facilities and services (MTF).
- Conduct suicide risk screening using the Columbia Suicide Severity Rating Scale brief screening for Military Service Members and Veterans.
- Collaborates with other community and veterans' agencies and programs providing services to veterans. The Veteran and Family Services case manager maintains primary responsibility for coordinating multi-disciplinary planning meetings for individuals served.
- Coordinates and co-facilitates Veterans and Family Support groups.
- Establishes and maintains comprehensive system for referral and networking of service and resource needs. Works to establish linkages between veterans served and naturally occurring supports in the family and community, as well as with other agencies/organizations.
- Assures health and safety needs of individuals served are addressed as needed, including medical, dental, psychiatric, psychological, physical therapy, occupational therapy, assistive technology, and environmental modification needs.
- Provides psycho-education to veterans served and families/care givers in case management related areas to promote better understanding of mental illness and substance abuse issues. Provides information on, and actively encourages individuals served and family members to participate in community and agency-sponsored support and advocacy groups.
- Assists with admission process and discharge planning to and from private and public psychiatric hospitals, including providing support and input to the emergency services providers and judicial system during commitment hearings, when appropriate. Coordinates discharge process, including assuring follow up services as needed.
- Maintains all communications, written or verbal, with respect and confidentiality and complies with HIPAA standards and regulations in accordance with respective CSB and federal guidelines.
- Travels to other locations as required and provides safe transportation according to AHCS policies and guidelines to individuals served as needed to implement the ISP.
- Provides and/or coordinates training for community and other providers on issues related services for veterans/family members.
- Develops and distributes approved brochures, flyers, etc to promote Virginia Wounded Warrior Program and events. Coordinates with AHCS staff to assure all materials contain accurate and appropriate information.
- Attends AHCS Crisis Services staff meetings and all mandatory training required by AHCS. Performs other related duties as assigned by supervisor and/or other management staff.

While performing the duties of this job, the employee will often sit, stand, walk, bend, stoop, and reach above their head. Use of hands for placing and pulling objects. The employee frequently lifts and or carries over 30 pounds. Specific vision abilities required by this job include close vision. Specific hearing required by this job includes normal tones. May travel less than 5%. Almost all travel is local. |
