Job Title: Case Manager, Project ELECT
Position Summary: The Case Manager works respectfully with a minimum of twenty (20) to twenty-five (25) families who are pregnant and/or parenting teens and their children, in a strengths model approach.  The Case Manager will mobilize resources, assist students to graduate or attain a GED, help students reach self-sufficiency, and integrate into the community. It is also the responsibility of the Case Manager to complete and forward all the necessary forms, attendance and grades information, and student data as required by established data protocols and deadlines.

Duties and Responsibilities:
1.         Working with a minimum of twenty (20) to twenty-five (25) families and showing dedication to recruitment and retention of eligible families.
2.         Completing a minimum of quarterly home visits with each enrolled family and providing two hours of individualized case management to each student every month.
3.         Developing a plan for each student through the use of Motivational Interviews as required by DPE every six months, with updates every three months.
4.         Completing childhood assessments and updating them as required.
5.         Exhibiting an ability to work with families becoming self-sufficient, along with teen parents, adults, children, community agencies, and diverse groups.
6.         Showing a capability to support the growth of consumers and help them set goals.
7.         Assisting in childcare when needed.
8.         Maintaining case files on all participants with clear documentation as directed.
9.         Nurturing and supporting families and peers.
10.       Providing resources and making all necessary referrals to agencies and professionals.
11.       Encouraging families to enroll and participate in groups and activities.
12.       Implementing and participating in weekly teen groups.
13.       Establishing, advocating, and maintaining relationships with providers, school districts, social service agencies, CYF, DPW, and other systems.
14.       Participating in reciprocal relationships based on respect.
15.       Completing paperwork in a timely fashion.  Computer knowledge preferred.
16.       Assisting in cleaning and maintenance of Center and school areas.
17.       Working collaboratively with Family Center and school staff.
18.       Participating in on-going training.

Qualifications:
1.         Associates Degree in psychology, counseling, child development, social work, family relations, education, human service field, guidance and counseling a plus with two (2) years of experience working with infants/toddlers and families required.
2.         Bachelor’s Degree in psychology, counseling, child development, social work, family relations, education, human service field, guidance and counseling a plus with one (1) year experience working with infants/toddlers and families preferred.
3.         Experience working with the parents, children, and school personnel in both home and school environments.  Community services experience preferred. 
4.         Ability to travel.
5.         Ability to comply with the AIU Conduct policy.
Additional Requirements:
  1. Current Act 114, Act 34, Act 151 and NSOR required prior to an offer (Must be dated within 1 year of hire date).
  2. School Personnel Health Record (Form H511.340) must be completed and received by Human Resources prior to hire date.
