Proof of Everify or Attestation
Please require if nurse is working on a compact license
Initial physical, updated annually attesting to the ability to meet the physical demands of the offered position.
Negative PPD, Quantiferon Gold or T-spot required within 1 year of start. Updated annually. Past Positive - clear Chest Xray within a year of relationship with Qualivis and annual TB Questionnaire.
True 2-step PPD required (the 1st within 90 days of start, the 2nd placed at least a week after the 1st is read). Must be updated annually.Positive PPDs - Clear Chest X-Ray and annual TB questionnaire required.
