ACE provides education, workforce development, violence intervention & prevention, reentry navigation and mental health services for marginalized individuals and families. Knowledge, Skills And Abilities
- General proficiency in navigating and using technology.
- Demonstrate a period of stability and success in the community following system-involvement and/or incarceration and are deeply connected and rooted in their communities.
- Prior experience working with individuals with system involvement, mental illness, SUD, or experiencing homelessness, and/or experience assisting community members in navigating health and social service systems.
- Ability to communicate and build rapport with youth.
- Ability to follow program protocols and instructions.
- Some experience with case notes documentation and/or case notes database system.
- Ability to work independently and with a team.
- Ability to manage time and prioritize duties and time sensitive tasks.
- Mentor, motivate, and empower youth participants to become positive members of their communities by establishing a trusting and open relationship.
- Conduct outreach and engagement of eligible youth for program enrollment.
- Assess participants at the point of enrollment and provide appropriate linkages and referrals to needed services through an individualized care plan.
- Conduct intake and exit assessments.
- Maintain contact with youth participants 2 times a week at minimum, one of which should be in-person.
- Document interaction by updating case notes and care plans.
- Develop relationships with service providers to refer and link youth participants for any supportive services that are not offered in-house.
- Identify community resources appropriate to meet youth participants needs and assist through appointment accompaniment, transportation assistance, and coaching.
- Attend all required trainings and events organized by the County to foster learning and community.
- Exit youth participants who have successfully completed service and/or who are no longer actively engaged.

- 5 days of vacation time (at one-year anniversary).
- Vacation will then accrue to 10 days vacation after completion of 2 years working at ACE and 15 days of vacation time after 5 years of employment.
