In this multifaceted position, you’ll play a key role in guiding patients and their families through the admissions process while also helping to promote our community within the broader Brockton area.
Your workday will be a mix of engaging fieldwork building relationships with local community partners, and hands-on collaboration at our facility. You’ll partner with our Director of Admissions to develop creative promotional strategies, meet with potential patients, and ensure a smooth transition of care. If you’re someone who enjoys meeting new people, solving problems, and taking initiative, you’ll thrive here. A background in nursing is a plus but strong clinical skills and knowledge of SNF level of care is acceptable.

Key Responsibilities :
Support the admissions process by assisting with documentation and patient / family communications.
Develop relationships with local healthcare providers, including case managers and discharge planners, particularly at Brockton Hospital and surrounding facilities.
Conduct patient screenings and review clinical information to determine admission eligibility.
Represent the community at health fairs, events, and other outreach opportunities.
Collaborate with the Director of Admissions to implement marketing initiatives and community engagement strategies.
Provide tours and serve as a resource for prospective residents and their families.
Keep organized records of referrals, outreach efforts, and admissions data.
Contribute ideas to enhance admissions workflows and improve the patient experience.
Qualifications :
Clinical background preferred (LPN or RN), with an understanding of patient care and rehabilitation services.
Previous experience in admissions, marketing, or healthcare outreach is a plus.
Familiarity with the Brockton area and its healthcare network is highly valued.
Strong interpersonal and communication skills with a natural ability to connect with others.
Self-motivated and adaptable, with the capacity to work independently and as part of a team.
Enjoys a balance of in-the-field engagement and on-site responsibilities.
Valid driver’s license and reliable transportation required.
Job Types : Full-time 32 hours a week.
