WHAT YOU WILL DO:
- Serve as the primary link between healthcare providers and our care services.
- Build and maintain strong relationships with case managers, discharge planners, resident care directors, social workers, physicians, hospitals, and other referral sources you may have.
- Assess potential patients' clinical needs and communicate those effectively to our care team.
- Educate referrers and their patients about our services and coordinate smooth transitions to our care team.
