What are the responsibilities and job description for the Director -Community Transitions (Full-Time, Hybrid, North Carolina Based) position at Alliance Health?
The Director of Community Transitions position oversees the development and implementation of best practices for members who reside in an institution or are at imminent risk of institutional placement to transition to community-based living. This includes developing high quality teams and strong partnerships with a myriad of stakeholders to carry out all the functions of in-reach, diversion and transition which are essential to successful community living.
The selected candidate must reside in the State of North Carolina and will be required to work from the Alliance office located closest to their residency, at minimum one day a week. The ability to travel across the Alliance catchment area to meet with community stakeholders and to attend in-person staff meetings at any of the Alliance offices as needed is also required.
Responsibilities & Duties
Oversee Unit Operations
- Establish overall goals and objectives, sets priorities, and determine assignments for staff
- Manage projects aligned with the Transition to Community Living Settlement and NC Olmstead Plan
- Prepare, present and/or write reports that contain data and solid findings and recommendations related to transitions, wraparound services and supports to executives, senior management, and external stakeholders
- Analyze information and data to identify target areas for improvement in in-reach, diversion and transition teams’ performance
Manage and Develop Staff
- Monitor and manage staff productivity
- Ensure managers are well trained in Alliance policies, procedures, and business processes
- Identify training needs as appropriate
- Ensure the In-Reach, Diversion and Transition managers have the needed tools to fulfill functions and support employees
- Provide ongoing coaching and mentoring to managers on new technology tools and practices, including change management practices, to support a learning environment to advance team skills
- Develop, revise and organize workflows and ensure managers understand their roles and responsibilities
Develop and Monitor Quality Transition Operations
- Work closely with Senior Director of Community Living, DHHS and other state partners in the successful outcomes for the DOJ Settlement and NC Olmstead Plan
- Manage and oversee all phases of TCL and any other identified Olmstead populations ensuring best practice guidelines for comprehensive and integrated care that will also apply post settlement period
- Assist with required TCL and any other state reporting requirements related to Olmstead
- Work cross collaboratively with other internal departments to develop community transition workflows for all disability populations
- Ensure compliance with contractual requirements and develop risk mitigation strategies as necessary
Minimum Requirements
Education & Experience
Bachelor’s degree in Human Services, Social Work, Public Health, or related field from an accredited college/university and seven (7) years of progressive experience with behavioral health population, with at least 5 years progressive management experience

Master’s degree in Human Services, Social Work, Public Health, or related field from an accredited college/university and five (5) years of progressive experience with behavioral health population, with at least 5 years progressive management experience
Knowledge, Skills, & Abilities
- Knowledge of and experience with Program development
- Knowledge of the Olmstead Settlement Agreement
- Knowledge of the 1915 service system.
- Ability to develop, lead, manage and report on complex multisystem projects
- Excellent problem solving, negotiation and conflict resolution skills
- Excellent skill in communicating, both verbally and in writing
- Ability to make prompt independent decisions based upon relevant facts and established processes
- Proficiency in Microsoft Office products or (such as Word, Excel, Outlook, PowerPoint etc.) is required
- Ability to establish and maintain effective working relationships with internal and external stakeholder
Salary Range
$98,800-$125,970/Annually
Exact compensation will be determined based on the candidate's education, experience, external market data and consideration of internal equity.
