What are the responsibilities and job description for the Community Health Worker position at Allmed Staffing Inc?
Large managed care company is looking for bilingual Community Health Workers-to educate members on health and wellness information to improve the well-being of individuals, families, and communities. This is a short-term contract position.
WORK LOCATION: FULLY REMOTE
PAY: $23.00
CONTRACT DATES: 3/07/25 - 5/16/25
HOURS: M-F, 8 am – 5pm MST
RESPONSIBILITIES
- Engage members over the phone to have a discussion about their health
- Enroll members into appropriate programs and services
- Conduct comprehensive member assessment that includes bio-psychosocial, functional, and behavioral health needs
- Utilize interviewing techniques and active listening to collect and retain member information
- Identify urgent member situations and escalate to next level when necessary
QUALIFICATIONS
- Highschool graduate with 3 years of experience working within the community health setting in a health care role OR Bachelor's degree in social work, licensed practical nurse (LPN), CNA/HHA
- Must have MS Teams and MS Outlook experience
- Must be bilingual in Spanish
- Clinical platform experience is required
