Objective:
Supports the operations, programs and activities for services provided under the NYSDOH Home and Community Based Services Medicaid Waiver for Individuals with Traumatic Brain Injury (TBI) and/or Nursing Home Transition and Diversion (NHTD) Waiver.
The Service Coordinator advocates and coordinates services for consumers on the TBI and/or NHTD Waiver Program(s). The Service Coordinator (SC) ensures all necessary waiver services are implemented and maintained in compliance with all relevant regulations and policies, and in doing so will meet the following obligations: Supports the operations, programs and activities for services provided under the NYSDOH Home and Community Based Services Medicaid Waiver for Individuals with Traumatic Brain Injury (TBI) and/or Nursing Home Transition and Diversion (NHTD) Waiver. The Service Coordinator advocates and coordinates services for consumers on the TBI and/or NHTD Waiver Program(s). The Service Coordinator (SC) ensures all necessary waiver services are implemented and maintained in compliance with all relevant regulations and policies, and in doing so will meet the following obligations:
Role & Responsibilities:
· Ensure all required documentation and semi-annual service reports are completed in accordance with deadlines and DOH/Waiver regulations and distributed to appropriate agencies and staff.
· Ensure SC files for each Waiver Participant are in order, updated, and maintained according to the Waiver regulations and Allwel policies and procedures.
· Meet face-to-face with participants minimally one time per month, and as much as needed, in order to gather information, counsel and advise Waiver Participant’s regarding their progress in waiver programs, satisfaction with waiver services, need for additional services, etc. Provide documentation for each face-to-face encounter as well as all contacts (telephone, email) to the participant or on their behalf. At least one face-to-face meeting will be in the participant’s home quarterly or as needed.
· Coordinate and facilitate Clinical Team Meetings as prescribed in the approved Service Plan, ensuring staff representation from each waiver service is present in-person or by phone/video, and that documentation is provided to show staff involvement.
· Ensure Waiver Participants’ confidentiality consistent and compliant with Allwel’s policies based on HIPAA.
· Maintain inventory of materials and equipment.
· Additional responsibilities to be assigned.
Education & Experience
· The provider shall have at a minimum, one (1) year of experience providing Service Coordination in human services and Master’s degree.
OR
· Be an individual with a Bachelor’s degree and two (2) years of experience providing Service Coordination in human services.
Skills & Abilities:
· Demonstrate the correct use of spelling and grammar.
· Ability to read, write, speak and listen. Ability to effectively present information to co-workers and supporters.
· Ability to perform basic math.
