What are the responsibilities and job description for the Community/Crisis Nurse position at Allwell Behavioral Health Services?
Utilizes the Nursing Process for primarily community based nursing services to the population of clients with mental health/substance use/physical health disorders. Provides health and safety education programs to team members, clients, group home operators, residential settings, and Allwell staff as indicated.
RESPONSIBILITIES/STANDARDS:
Provides primarily community-based health/safety to individuals, families, and clients.
- Provides community linkage and coordination with health care providers, Allwell PMP, and other services as indicated.
- Reviews and completes health assessment, manages health care needs of clients, as well as follows-up on health related concerns brought to the Health and Safety Committee.
- Review and evaluates medication administration and compliance to various sites (home, community, group homes) and coordinates/trains group home operators.
- Assists clients with maintaining health and medical needs within the community by developing, implementing, and evaluating these needs utilizing the treatment plan.
- Provides nursing services after routine hours as needed.
Provides in-service education/training on health/safety issues for Allwell staff, clients, and/or group home operators, and as indicated by the needs presented.
Maintains a service conversion ratio of 100% as documented on the monthly production report.
Completes all documentation related to providing and billing for services via Allwell guidelines. Clinical documentation complies with Allwell Quality Improvement guidelines.
Complies with clinical supervision requirements.
Has access to and use of as well as may create Protected Health Information (PHI) of clients through verbal, written and electronic communications as recorded and contained in Allwell’s electronic record system that the employee and his/her supervisor have identified on attached document entitled:
