Full-time, PRN & Weekend positions available in Lafayette, Indiana. Sign-on bonus available. Minimum start pay is $22.00/hour. Responsibilities include:
- Implementing nursing care plans under RN direction.
- Providing home-based patient care per physician orders.
- Reporting patient status changes to Home Health RN.
- Conducting psychosocial assessments and collaborating with care teams.
- Documenting patient care and developing discharge plans.
- Educating patients and caregivers about resources.
- Associate's Degree or Diploma in Practical Nursing.
- 3 years of acute or long-term care experience required; 1 year in home health preferred.
- Must have LPN license, CPR certification, driver's license, and auto insurance.
- Travel required up to 75%.
